A domestic partner is someone who is not a relative and has lived with you for at least six months in a committed relationship. Domestic partners are eligible for benefits. For more information, contact the Five Below Benefits Support Center at 855-952-3569.
You must enroll within 30 days of your hire date!
If you miss this deadline, you will not be eligible to enroll in benefits until the next Open Enrollment period (usually in the fall) unless you experience a Qualifying Life Event during the year. For more information about Qualifying Life Events, click the “Midyear Changes” tab at the top of the page.
A Summary of Benefits and Coverage (SBC) has been designed to assist you with better understanding the coverage being offered to you, and to allow you to compare coverage options. The SBC is available on www.fivebelowbenefits.com. A paper copy is also available, free of charge, by calling 855-219-4530.
